Prep 2nd day Patient Report of Findings – Purple Folder

Purpose:

  • Review the New Patient’s case and determine whether they are a 3,6,9, or 12 month Careplan.

Use the ASC Excel Sheet. This Excel Sheet will save time for you to properly and accurately create a care-plan for your patients. The beauty of this form is that it creates care-plans specific to each patient.
